The American Heart Association’s Center for Health Technology and Innovation and GoMo Health have teamed up for the prevention and treatment of cardiac conditions and associated comorbidities

The devastating effects of cardiovascular disease are the driving factor behind a new collaboration between GoMo Health and the American Heart Association. Consider these facts:

  • Cardiovascular disease is our nation’s top killer; accounting for about 1 of every 3 deaths in the U.S.
  • About 2,300 Americans die of cardiovascular disease each day, an average of 1 death every 38 seconds.
  • About 92.1 million American adults are living with some form of cardiovascular disease or the after-effects of stroke.

GoMo Health has joined the American Heart Association Center for Health Technology and Innovation’s (CHIT) Innovators Network. We are integrating the Association’s digital care plans and educational content into the award-winning suite of Concierge Care® solutions.

The result: personalized, actionable and interactive resources for patients that are delivered at the preferred time, frequency, and tone that best matches their lifestyle.

As a telehealth care coordination solution, Concierge Care is deployed in partnership with hospitals, health plans, skilled nursing, life sciences and pharmaceutical companies nationwide, touching millions of lives daily and supporting patients across the continuum of care as they manage complex chronic conditions like cardiovascular disease. Concierge Care supports CMS value-based outcome and cost measures of reducing readmissions and unnecessary ED visits.

In addition to remote telehealth usage, Concierge Care is designed to be utilized in-hospital to reinforce treatment protocols and to simplify and increase compliance with discharge instructions. A modular solution comprised of outcomes-focused programs, Concierge Care takes an integrated treatment approach, addressing the overall health and lifestyle needs of the complete person, not just their diseases.

“The American Heart Association is passionately driven to reduce deaths, improve outcomes, and increase prevention of cardiovascular disease. We value innovators like GoMo Health who embrace those objectives and are focused on behavior-based, patient-centric care leading to measurable and sustainable population health.”
