Sourcewise & GoMo Health Collaborate to Win Prestigious Aging Innovations Award from USAging

Asbury Park, NJ, September 22, 2023: GoMo Health and Sourcewise are thrilled to announce that MyPlan™, their joint product collaboration, received a 2023 Aging Innovations Award from USAging, one of the highest honors presented by USAging to its members. MyPlan, an interactive program that connects care managers to caregivers to provide real-time assistance and support, was among sixteen local aging programs to receive innovation honors during the USAging’s 48th Annual Conference and Tradeshow, July 16-19, 2023 in Salt Lake City. The innovative MyPlan program was initiated through grant funding from Dr. Venus Wong and Dr. Sarah Rosenbaum of the Clinical Excellence Research Center at Stanford University.

The 2023 USAging Aging Innovations & Achievement Awards recognize USAging’s Area Agency on Aging (AAA) and Title VI Native American Aging Program members that have found new and innovative ways to support older adults, people with disabilities and caregivers as they age in place – living in their homes and communities.

“The Aging Innovations & Achievement Awards are a timely reminder of the evolving role USAging members play in the planning, development, coordination and delivery of aging and other home and community-based services in nearly every community in the United States,” said USAging CEO, Sandy Markwood. “The AAAs and Title VI programs we honor today have set a standard for innovation and creativity in helping vulnerable older adults live with independence and dignity in their homes and communities,” Markwood added. “USAging is inspired by the example they have set for others in the Aging Network.”

Sourcewise MyPlan offers caregivers real-time chat (text) capabilities with a Sourcewise Caregiver Care Manager, and access to individualized resources and informative articles through a secure dashboard accessible by mobile phone or web browser. Leveraging digital technology, MyPlan enables the Sourcewise Caregivers Network to support more caregivers by triaging caregiver needs through “bot” technology, extending their reach and impact with improved access to service quality and personalized assistance—all the while tracking health-related outcomes.

Regarding the impact of MyPlan, Del Pinal continued, “After three months of enrollment in MyPlan, informal caregivers report reductions in their levels of stress and depression. Data shows that MyPlan expanded the caregiver’s knowledge of resources; for example, their awareness of reliable transportation increased by 12%, levels of food insecurity decreased by 10%, and their overall levels of met needs increased by 30%.”
