Breakthrough Early Intervention Digital Therapeutic to Support Children and Families

Amerigroup Community Care of Georgia helps improve health care access and quality for more than 500,000 Medicaid recipients in Georgia by developing innovative care management programs and services. Through health education programs, members are empowered to choose and sustain a healthy lifestyle.

Amerigroup and GoMo Health partnered to launch an Early Intervention Mobile Concierge Program for Families with an autism spectrum diagnosis. This program supports members with autism and their caregivers by ensuring they are getting the resources they need at the earliest point of intervention, and by providing psychosocial support to ease the strain on caregivers.

The program was built with a unique care management component to assist families in foster care to ensure support is delivered to the current caregiver. The program also ensures adherence with appropriate autism screens and milestone tracking for all plan members with children under four.

Early Detection Program:

  • Increase well-visit appointment completions to ensure appropriate pediatric autism screenings are completed
  • Reduce barriers such as transportation, language, childcare to improve well-visit attendance
649 self-reported barriers to well-visit appointment compliance; resolved by care coordination team3,238 escalations to Amerigroup care coordinators who were able to assist caregivers in getting additional follow up after a well-child visit

Program Components

  • Care Communications:

    Upon enrollment, participants receive text messages to support their autism spectrum journey both physically and emotionally, with some including links to a library of resources (see Care Companion). Topics include caregiver stress support, foster care, ADL & Education Support and general autism for both adults and children.

  • GoMo Chat:

    Much like a typical text conversation, GoMo Chat allows (secure) live messaging between participants and their care team, enabling in-the-moment, on demand conversations.

  • Tablet with a heart and information

    Care Companion:

    This cloud-based learning management system(LMS) provides vetted educational content that is personalized by participants’ self-expressed priorities and interests, structured into easy-to-follow sections to promote independent education, learning and self-care management.
