Integrated Digital Care Platform Enhancing Health Outcomes for Under Served Communities
Building on their established presence in Coney Island, New York, the Community Health Initiative (CHI) Health Center, a Federally Qualified Health Center (FQHC) since 2015, has grown to become a trusted community asset, serving thousands of residents in Coney Island and South Brooklyn. As a fully integrated clinic, CHI’s multidisciplinary teams deliver a holistic approach to comprehensive care, including preventive health services, behavioral health services, specialty care, and supportive services.
To extend their healthcare services, CHI launched a new affiliated provider, EveryYou Health. The introduction of EveryYou Health, which includes the PrEP (pre-exposure prophylaxis) Concierge program, represents the CHI commitment to expanding and enhancing healthcare services for underserved and vulnerable communities. This integrated patient engagement and management solution, created and powered by GoMo Health, provides personalized guidance and timely support throughout patients’ healthcare journeys.

Marketing the Vision: Building the EveryYou Health Brand
To create the EveryYou Health brand, CHI leveraged the creative services of GoMo Health, which encompassed a comprehensive approach to brand development and marketing. This included designing a new logo and brand identity, establishing brand guidelines, and creating marketing materials, including flyers, posters, and digital content. Additionally, GoMo Health designed and developed EveryYouHealth.com, ensuring it served as a user-friendly platform for health information, services, and program enrollment.
The marketing strategy was to drive awareness and enrollment in the PrEP Concierge program within the local community, with a focus on serving the LGBTQIA+ community. The dedicated website enables enrollment into the PrEP Concierge program, seamlessly integrating patient registration into the digital experience
By combining targeted digital marketing campaigns, community outreach, and engaging content, CHI effectively communicated the benefits of EveryYou Health and the PrEP Concierge program to the target population. This holistic marketing approach ensures that the program reaches those who need it most, enhancing engagement and improving health outcomes for vulnerable communities
Program Components
Personal Concierge:
Upon enrollment, participants receive text messages that nurture, guide, and support their healthcare journey. Some messages contain links to online resources with additional educational information in the form of interactive surveys, and videos (see Care Companion). Topics include condition coping, mental health, daily life management, nutrition, refill reminders, and medication management.

Care Companion:
This cloud-based learning management system (LMS) provides vetted educational content that is personalized by participants’ self-expressed priorities and interests, structured into easy-to-follow sections to promote independent education, learning and self-care management.

Secure Data Collection and Reporting:
Engagement and experience data is deidentified, analyzed, and reported back to the organization for ongoing quality, clinical delivery and service improvement.
