Asbury Park, NJ – In support of remote care coordination and population health management resources for the aging in place, post-acute rehab and senior living patient populations who can directly benefit from assistance managing their health care, GoMo Health has donated dollars to the Village to Village (VTV) Network, furthering their efforts to launch a virtual health program for participating Villages, their residents and caregivers.

“GoMo Health is dedicated to enabling people to live healthier lives and removing any access barriers to tools to support them.  Through our Concierge Care® solution, we deliver resources directly to people no matter where they are, in the way they wish to receive the information”, says Bob Gold, CEO and Behavioral Technologist, GoMo Health.  “In direct alignment with our mission, the VTV Network was established to coordinate access to affordable services for Village residents.  We elected to support the VTV Network because of their total focus and dedication to helping Americans live healthier, happier lives with dignity. Local people want to help their neighbors and VTV provides the structure and support to make that happen quicker and more effectively.

Patient and family engagement is one of six core components of Centers for Medicare and Medicaid Services (CMS) Quality Strategy Goals and plays a crucial role in decreasing hospital readmissions.  The Institute for Healthcare Improvement (IHI) identifies a cornerstone of the care model as informed and active patients.  GoMo Health’s Concierge Care® solutions drive reduced readmissions, cost of care reduction and increased patient satisfaction by personalizing engagement for the patient and their caregiver.
