Helping mothers through pregnancy and baby’s first 15 months
The program engages the expectant mom by providing personalized messaging based on her physical and emotional condition. This allows us to engage, educate and support the soon-to-be new mom throughout the course of her pregnancy with timed messaging, information and support that considers her stage of pregnancy, physical and mental health, circle of support and other behavioral and environmental factors.
Beginning with the objectives of improving the health of the mother and baby and reducing complications and costs, we obtain baseline data from each woman through an interactive mobile-based survey. Using decision tree logic, we weigh each response and stratify individuals into a low, moderate or high-risk category — the greater the risk, the more messaging they receive. Many of the messages link to videos and quick pages that provide more detailed information on specific topics.
But we don’t stop there. Once we’ve partnered with mom during her pregnancy, our Baby’s First 15 Months program takes over to ensure the baby’s healthcare needs in the crucial first stage of life are efficiently and easily managed – with things like nutrition, vaccinations and well visits kept on track during this critical period in an infant’s development.
Combatting high rates of premature birth
Currently, the U.S. has one of the highest rates of premature birth amongst high-income countries. Concierge Care is working hard to address this situation.
In fact one of our clients, the University of Nebraska Medical Center (UNMC) began a new prenatal study aimed at providing high-risk mothers with important health information [Read more about the UNMC study here].
