We’re excited to announce that GoMo Health has partnered with Planetree to create The Planetree Concierge. The Planetree Concierge is a new form of personalized engagement technology that sends “in-the-moment” messaging and supportive content directly to mobile devices, to support patients and clinical caregivers.
The Planetree Concierge works for both the patient and the clinical care team by meeting people where they are.
- For patients and their families: A personal health educator and care coordinator, Planetree Concierge guides clinical care with user-friendly, step-by-step tips, directions and reminders.
- For clinical care delivery team: Planetree Concierge is a clinical “concierge” supporting the role of a case manager – providing transitional care support to patients where, when and how they need it.
The Planetree Concierge has been designed to reinforce the patient-centered guiding principles, verbal and physical staff training and best practices for consistent communication and empathy across the continuum of care that Planetree provides to hospitals. It is a highly cost effective, evidence-based and scalable method to measure, benchmark, analyze, and improve.
Through personal engagement and active participation, a relationship is built, and patients are more receptive to directives, and more inclined to reciprocate and complete desired outcomes.
