GoMo Health will be integrating the Association’s digital care plans and educational content into Concierge Care®. The result: personalized, actionable and interactive resources for patients that are delivered at the preferred time, frequency, and tone that best matches their lifestyle.
Cardiovascular disease, listed as the underlying cause of death, accounts for about 1 of every 3 deaths in the US. About 2,300 Americans die of cardiovascular disease each day, an average of 1 death every 38 seconds. Furthermore, about 92.1 million American adults are living with some form of cardiovascular disease or the after-effects of stroke. The devastating effects of cardiovascular disease are the driving factor behind the innovations that GoMo Health is creating in cardiac condition management in 2018.
As a telehealth care coordination solution, Concierge Care is deployed in partnership with hospitals, health plans, skilled nursing, life sciences and pharmaceutical companies nationwide, touching millions of lives daily and supporting patients across the continuum of care as they manage complex chronic conditions like cardiovascular disease. Now, Concierge Care will be deployed with the expertise of the digital care plans and educational content that is developed by the American Heart Association.
