Meeting People Where They Are Using Telehealth Digital Therapeutics

March 25, 2021 (Asbury Park, NJ) –Crossroads4Hope and GoMo Health® announce MyGo2Support, a unique collaboration to deliver individualized psychosocial telehealth cancer care that extends Crossroads4Hope’s previously in-person support programs directly to people’s homes. MyGo2Support serves cancer patients, families, caregivers, survivors, and those in bereavement throughout New Jersey.

Cancer patients and caregivers face a unique set of challenges. MyGo2Support is the first psychosocial oncology platform that is designed to plug-in to cancer center, oncology practice, and health plan clinical workflows – streamlining and reducing time and cost by automating referrals and consistent distribution; data collection; triage of distress screenings; and other required behavioral health assessments (i.e., PHQ2, PHQ9). The program provides real-time reporting and escalation to the practice or hospital upon early detection of a potentially avoidable adverse event, ED visit, or readmission.

The program is structured in a membership model that delivers personalized content based on the specific needs and circumstances of the patient. The Crossroads4Hope MyGo2Support program includes Personal Concierge mobile messaging and content delivery, along with GoMo Chat, an in-the-moment bi-directional chatbot that allows direct engagement with people affected by cancer and their caregivers to better support them throughout their cancer journey. Crossroads4Hope’s clinical expertise in social work, public health, and child life have informed GoMo Health’s proprietary AI model to create a safe environment with social work oversight, providing customized support and interventions to meet individuals where they are in their journey.
