Heart Care Beyond the Doctor’s Office – James River Cardiology Concierge Program

James River Cardiology is a multi-location Virginia-based cardiovascular practice providing patients with the latest technologies and innovationswith passion to deliver a clinically integrated approach to whole-person care beyond the four walls of their clinics.  

GoMo Health and James River Cardiology joined forces to launch a Personal Concierge outpatient digital therapeutic for cardiac patients (and their caregivers) undergoing a cardiac catheterization and/or percutaneous coronary intervention (PCI). The program engages patients and caregivers both before and after their PCI/cardiac catheterization to increase compliance with pre-and post-procedural care, promote advised lifestyle modifications, and support patients in their day-to-day lives. 

Personal Concierge Goals 

  • Reduce procedure cancellations/postponements due to patients not following pre-procedure instructions 
  • Decrease tobacco use in participants who smoked upon enrollment    
  • Improve medication adherence 
  • Increase patient activation measures (PAM) 
  • Decrease post-procedure complications 
  • Decrease unnecessary ER visits and hospital readmissions   
  • Improve patient-reported wellbeing and quality of life (QOL) 

Getting to the Heart of Behavioral Cardiology 

Personal Concierge uses the BehavorialRx® science of engagement to communicate via mobile messaging and interactive “conversations” to guide patients throughout the continuum of their care.  

Nurturing an ongoing, interactive patient relationship has proven to decrease complications and increase patient compliance leading up to and following the procedureGoMo Chat is an on-demand bi-directional chatbot that enables triage of patients’ self-expressed challenges, responding as appropriate through automation, and escalating to a live care coordinator at James River Cardiology when needed to provide direct support to avoid potential adverse events.
