GoMo Health Mom’s Heart Matters: A Proven Solution to Georgia’s Maternal Mortality Crisis

A Digital Intervention Platform Created for Postpartum Moms is Improving Outcomes and Saving Lives

Atlanta, Georgia, August 15, 2024: Liberty Regional Medical Center, in partnership with GoMo Health, celebrated the success of the Mom’s Heart Matters platform at a community event on August 14, 2024. Mom’s Heart Matters is an interactive digital intervention that leverages behavioral and cognitive science to improve perinatal wellness and reduce the rate of hypertension-related mortality. Since the launch of this patient-influenced intervention, Mom’s Heart Matters has tracked numerous evidence-based cases of reduced adverse events and life-saving interventions.

“As one of two critical access hospitals in the state of Georgia that still deliver babies, we are dedicated to improving the high maternal mortality rate for the state, and the community that we serve. It has been amazing to see the life-saving impact of the Mom’s Heart Matters program over the past year, and how we are able to reach these moms immediately in their moment of need and get them the help that they deserve.” said Tammy Mims, the CEO of Liberty Regional Medical Center.

The platform includes remote patient monitoring (RPM) that utilizes Bluetooth enabled blood pressure cuffs to collect real-time patient data and deliver it to the care team to enable immediate response as/if needed. This combines with delivery of resources that are personalized to each participant to address clinical needs and social drivers of health for better overall care.

In the initial pilot, there was a 95% retention rate among all enrolled participants. Of the participants opted into remote blood pressure monitoring, 23% had escalations of which 100% were addressed with real-time interventions.. In addition to blood pressure monitoring, the platform screens for depression (PHQ-2 and PHQ-9), maternal functioning (Barkin Index of Maternal Functioning), and substance use (SBIRT) to identify each moms’ needs and connect to necessary resources.

Bob Gold, Chief Behavioral Technologist and Founder, GoMo Health, says, “Postpartum is one of the most critical times for a mother and her infant, setting the stage for long-term health and well-being. Mom’s Heart Matters sets families up for success with a strong support system, ensuring mom is safe and healthy both physically and emotionally.”

The innovative platform advances health equity by creating a safety net that seamlessly integrates physical, behavioral, and social drivers of health with the service community designed to care for them.

The initial pilot at Liberty Regional Medical Center was a collaboration by GoMo Health, Annie E. Casey Foundation, Amerigroup, Georgia Family Connection Partnership, the Get Georgia Reading Campaign, and the Georgia OBGyn Society. GoMo Health and its collaborators are proud to pave the way for a healthier postpartum experience.
