Focusing on the complex needs of underserved populations, CPC aims to reduce health disparities and improve outcomes.
CPC Integrated Health enhances their participant’s recovery experience with the Recovery Pathways personal concierge program. This program coaches users with personalized content that creates and nurtures positive behaviors and empowers healthy daily living.

Nurturing Members Through Their Journey
The long-term program implements personalized engagements focused on supporting members during their enrollment to increase consistent attendance at sessions as well as providing resources to support and extend the techniques shared in live sessions.
The mobile-based program delivers messages directly to the member with an overall goal of encouraging increased activation, self-help, and independence in each participant’s daily life.
In-the-Moment Engagement at Members’ Fingertips
Through periodic surveys and assessments, the care team is immediately notified if participants need additional support and can adjust individual care plans as necessary to ensure optimal outcomes.
