Combatting a national crisis for providers and patients
Central Jersey Family Health Consortium, Inc. (CJFHC), is a leading private non–profit 501(C)3 organization licensed by the New Jersey Department of Health and part of a regionalized maternal and child health (MCH) system. CJFHC works to improve the health of women of childbearing age, infants, and children in the region through the collaborative efforts of member hospitals, providers, and consumers.
CJFHC and GoMo Health partnered to create two different programs that support prenatal and postpartum women with opioid use disorder (OUD) and young women of childbearing age.

Creating a personalized learning management system for healthcare providers
Utilizing the GoMo Health Care Companion, CJFHC is able to support their team with the Provider Concierge; a personalized interactive learning management system designed to support healthcare providers with education and resources to advise patients, families, and caregivers on best plan of care and available resources.
Healthcare providers enroll in the Provider Concierge by completing a simple onboarding survey which customizes the content they’re able to access. Content in the CJHFC Provider Concierge is personalized and organized by:
- Category of healthcare provider: Doctor, staff nurse, advanced practice nurse (APN), midwife, or social worker
- Healthcare provider’s specialty: OBGYN, maternal fetal medicine, behavioral health, neonatologist, family/general practitioner, pediatrics
- Location: Zip code & county
A go-to-destination for healthcare provider education
Based on the specific needs of each provider, the Companion contains educational resources related to:
- Medication Management: Content to establish which of the 2 FDA-approved drugs should be recommended based on patient lifestyle, establishing a birth plan prior to labor/birth, information on replacement therapies to offer patients to avoid withdrawal
- Resources: Mental health resources, recovery support groups, self-help groups, resources specific to veterans, lifestyle support, and smoking cessation resources
- Patient Care Plan Checklist: Questions about patient’s physical and mental health and how often medication assisted therapies (MAT) are being used, etc.
- Shareable Patient Content: Content that can be shared with the patient to educate and inform them about opioid use
An interactive mobile concierge for women with Opiod Use Disorder (OUD)
In addition to supporting healthcare providers, CJFHC and GoMo Health worked together to create a program designed for prenatal and postpartum women with OUD called The Maternal Child Health Personal Concierge.
To enroll in the Maternal Child Health Personal Concierge, patients or their healthcare provider simply fill out an onboarding form which includes basic demographic information such as stage of pregnancy or the baby’s birth date. Based on the mother’s responses to this survey, they will receive customized messaging which is personalized to their unique stage of motherhood.
Customized content delivered directly to mothers’ mobile devices
As early as six weeks prenatal, mothers will receive content related to their pregnancy. After the mother gives birth, messaging related to maternal and child health progresses until the child is 15 months old. A patient may receive a care message as often as six times a week.
Messages are designed to educate and engage with patients and are customized for prenatal and postpartum moms. Message tracks include:
- Prenatal: Message tracks include medication management, resources, lifestyle support, healthy eating, check-up reminders, and more.
- Postpartum: message tracks include medication management, mother/baby care plans, lifestyle support, safety, growth and health milestones, post-birth warning signs, and more.
End-to-end support for peer educators
The CJFHC PHE Personal Concierge is a post-classroom education program, designed to guide and support the participants by reinforcing the PHE training and activate their healthy lifestyle. For three months post-training, the PHE Personal Concierge reinforces the PHE guiding principles by driving increased learnings and self-confidence, as well as engendering higher active participation and ROI to the PHE program.
Through the PHE Personal Concierge program, Care Messages™ are delivered each week to engage, support, and educate participants using the best plans of care and available resources. Messaging delivered includes:
- Preconception health and family planning
- Nutrition
- Physical activity
- Inspiration and motivation
- LGBTQ health
- Effects of drug and alcohol use
