Four Seasons Nursing and Rehabilitation Center is a 270-bed facility in Brooklyn, NY, known for their exceptional inpatient and home-based care services. Four Seasons provides long-term care, short-term care, hospice, adult day health care and hemodialysis.

GoMo Health and Four Seasons partnered to enhance patient post-discharge care with a condition-specific readmission reduction program designed to improve transition to home-based care and improve patient health. The program is designed for both patients and their caregivers.

Personalized healthcare information delivered to your fingertips

Upon enrolling in the Four Seasons Concierge Care program, patients receive up to three Care Messages™ per week via email or text message.  These messages are designed to help patients understand their care plan and better manage their health at home. They can include health-management tips, condition-specific education, general wellness information, reminders, and escalation paths should their health deteriorate. Care Messages can also be sent to a caregiver or family members to assist patients with their in-home care.
