Using cognitive connection to convert patients into members
Since 1968, Bridgeway Rehabilitation Services has been providing rehabilitative care to people who suffer from mental illness by delivering comprehensive wellness services to guide them on a successful path to recovery and independence. Bridgeway partnered with GoMo Health to leverage our BehavioralRx® science and Personal Concierge Care™ system to shift the self-perception and mindset of program participants.

Create an improved, personalized, and sustainable approach to the continuum of remote care coordination and engagement within the behavioral health youth population that is either homeless or at risk of homelessness.
Increasing attendance and participation by creating a sense of membership
Every Day Matters seeks to engage members, drive participation, and increase session attendance by promoting the concept of belonging and self-worth.
Members receive a cadence of messages that correspond with the days of the week they are scheduled to attend and their probability of attending, based on past individual behavioral trends. These Care Messages include content designed to motivate and empower members to take a proactive approach to manage their health, future, and happiness.
In-the-moment engagement at member’s fintertips
Members can text in a simple keyword to receive the specific content that will help them at that moment.
PEER: An excerpt of a success story from an anonymous peer.
SMILE: A joke or feel good message to make them smile.
FENCE: A motivational message and a reason why they should attend, for days when they are “on the fence.”
MOOD: A motivational message for when they are feeling down, depressed, or angry.
SPIRIT: Faith-based, spiritual content. HALT: Messages focus on redirecting negative behavior when you are hungry, angry, lonely, or tired.
RELAX: Messaging focused on behavioral health stigma. SONG: Members can text in to receive a link to a feel-good song.
Improving behavioral health outreach to at-risk youths
Designed to support SAMHSA youth outreach and initiatives, Your e-HOST Connection embeds GoMo Health Concierge Care as a mobile, virtual care coordinator and life coach for at-risk individuals aged 16-24 living with serious mental illness, serious emotional disturbance, and/or co-occurring substance use disorders.
Enabling communities to streamline recovery services
Your e-HOST Connection and Concierge Care interfaces between Bridgeway Rehabilitation Services case managers, local police departments, and hospitals to enroll persons served into the program. The program provides treatment services, peer support, and other recovery services and linkages to sustainable permanent housing to enrolled individuals.
