Four Seasons Nursing and Rehabilitation Center is a 270-bed facility in Brooklyn, NY, known for their exceptional inpatient and home-based care services. Four Seasons provides long-term care, short-term care, hospice, adult day health care and hemodialysis.
GoMo Health and Four Seasons partnered to enhance patient post-discharge care with a condition-specific readmission reduction program designed to improve transition to home-based care and improve patient health. The program is designed for both patients and their caregivers.

A 90 day post-discharge program designed to reduce readmissions
GoMo Health implemented a Personal Concierge™ pilot program for Four Seasons Nursing and Rehabilitation Center specifically focused on readmission reduction. The program provides five personalized plans-of-care, deployed for 90 days post-discharge:

Personalized healthcare information delivered to your fingertips
Upon enrolling in the Four Seasons Concierge Care program, patients receive up to three Care Messages™ per week via email or text message. These messages are designed to help patients understand their care plan and better manage their health at home. They can include health-management tips, condition-specific education, general wellness information, reminders, and escalation paths should their health deteriorate. Care messages can also be sent to a caregiver or family members to assist patients with their in-home care.
