As an innovative collaboration between the city of Trenton’s two hospitals, St. Francis Medical Center and Capital Health, the Trenton Health Team came to GoMo Health for community outreach, health improvement, and patient education solutions.

Our Goals
To improve health education, resource awareness, care delivery, and access for Trenton’s uninsured, underserved, indigent care patient population.
Promoting in-the-moment engagement through technology
Using GoMo Health’s in-the-moment engagement tools, Trenton Health Team disseminates weekly motivational messages and helpful health management information via text (SMS) message with links to videos and mobile web pages in both English and Spanish.
