Promoting improved lifestyle engagement and communication
Formed in 2012 by independent family physician practices in Northern New Jersey, Vanguard is dedicated to striving for excellence in patient-centered care, reducing healthcare costs, and achieving high-quality patient satisfaction measures. Vanguard’s physician leaders hold positions as Medical Residency Directors, medical school faculty, and serve as Chairs of hospital departments. Vanguard physicians include two past presidents of the New Jersey Academy of Family Physicians, and all Vanguard providers value the role of mentorship.
GoMo Health and Vanguard have partnered to support unique programs that create more effective lifestyle engagement and improve communications between patients and health care providers.

Utilizing GoMo Health’s Personal Concierge™ solution, Vanguard Concierge is an extension of care provided to patients with behavioral health conditions. Through the program, Care Messages™ are delivered 3-4 times per week to engage with patients and allow them to manage their condition proactively.
LIFEMAP™ is GoMo Health’s central method for health management and care coordination for type 2 diabetes patients. Through the program, patients have access to their individualized LIFEMAP, delivering a unique schedule of Care Messages and reminders to report on their glucose levels.

Vanguard implemented the following Concierge Care components into their programs
Sending Care Messages™ and useful information when it’s needed most
Patients enrolled in both programs receive a variety of Care Messages that can be one-way or interactive in nature. These messages offer support and useful information, nurturing patients in their self-care while promoting engagement with Vanguard care coordinators in real-time.
Access content anytime with the Care Companion™
The Care Companion is a cloud-based personalized learning management system or mobile library that supports each individual. Some Care Messages link patients directly to the Care Companion, where they can access additional program content at any time. The Care Companion provides behaviorally based, vetted content that is sequentially structured into sections and topics to promote independent learning and patient activation.
Real-time communication with GoMo Chat™
GoMo Chat is a HIPAA compliant, two-way communication tool that gives providers the ability to easily communicate with patients in a private, secure environment. Vanguard uses GoMo Chat to view the details of program-triggered patient escalations and follow up with the patients regarding their feedback submitted through the program.
GoMo Health leveraged its proprietary science, BehavioralRx, to build the program and determine the engagement strategy, approach, and content delivered.
