GoMo Health partnered with 360 Smarter Care to leverage their artificial intelligence capabilities to further customize the GoMo Health evidence-based health management program for Victaulic’s employees. Their joint goal was to provide a consistent, personalized and highly cost-effective way to engage with employees and motivate them to adhere to care plans and better manage their often co-morbid chronic conditions.

Personalized Condition Management
Employees are able to choose up to two conditions to receive ongoing engagement and communication via text messages, bi-directional surveys/assessments and links to resources. This condition specific list was developed by Victualic, and based on costs associated with management of the costliest conditions. With this early involvement, employees are not only influencing their care management plan, but are also empowered to take control of their chronic conditions and maintain a healthier lifestyle, through incremental breakthroughs in their daily lives and closer adherence to their physician’s care plan.

Victaulic employees also have access to 360 Smarter Care’s proprietary Smarter Search tool which allows employees to easily and conveniently find quality care and save money by going to a ‘Smarter Search approved’ provider for routine care. The search system acts as a one stop shop for healthcare needs, guiding insured employees to lower out-of-pocket, in-network facilities with an easy to navigate tool that simplifies a traditionally complicated process.
